Case Management
Our case manager offers comprehensive services and support to our residents who are in need of assistance with improving their quality of life and further enhancing their integration in the community.
The primary goal of case management is to connect residents with needed services. Services include linkage for medical and clinical oversight including, social, educational, insurance, employment, financial, and other services. Provisions of these services will help you function as independently as possible upon discharge to home or independent living.
Case Management services include but are not limited to:
- Individualized, person-centered treatment planning
- Face to face contact between you and our case manager
- Linkages to social services, transportation, vocational and employment programs, medical, and dental care
- Disability application as needed
- Focus on individualized care and needs
Daily Living Skills:
Often times with addiction, individuals struggle with the ability to care for themselves. During your time at Promise House/Palmerton Place you will learn a variety of independent living skills like:
- Time Management: Maintaining a daily schedule
- Meal planning and a healthy diet
- Taking care of personal hygiene and living space
- Understanding financial independence: Personal banking, account balance
Vocational/Educational:
As part of our Reintegration Program, with guidance and support from our Vocational Counselor, residents will have the opportunity to be involved in one or two of the following areas:
- Obtain part time job
- Career exploration
- Job searching
- Community volunteering
- Attend West Seneca West Senior High School
- Complete a TASC exam
- Receive homework help
- ACCES-VR
- Higher level education
