Case Management

Case Management

Our case manager offers comprehensive services and support to our residents who are in need of assistance with improving their quality of life and further enhancing their integration in the community.

The primary goal of case management is to connect residents with needed services.  Services include linkage for medical and clinical oversight including, social, educational, insurance, employment, financial, and other services.  Provisions of these services will help you function as independently as possible upon discharge to home or independent living.

Case Management services include but are not limited to:

  • Individualized, person-centered treatment planning
  • Face to face contact between you and our case manager
  • Linkages to social services, transportation, vocational and employment programs, medical, and dental care
  • Disability application as needed
  • Focus on individualized care and needs
